I think my friend needs to change his medication

I have a patient with blood sugars between 250 to 500 and I cannot get the provider to change his medication. They refuse to give them a meal time insulin or Trulicity because their A1c is 8. They have told me several times that an A1c of 8 for a 43 year old man with HTN( it is under control)  is good and the literature tells you not to change their therapy.  I am worried that they may go into Diabetic Ketoacidosis so I want him to start using ketone strip to see if his ketones are spiking. I am worried sick about him and cannot understand how a provider could think that someone walking around with a blood glucose of 500  is fine and doesn't need their medication adjusted to work with this  high blood sugar. I need help !!!!Should I tell them to get the Ketone strips behind the providers back? I cannot get him to go to another provider because they think they are wonderful. Does anyone have any suggestions????
